2.1 Lesson Notes — History of Nutrition and Dietetics
Introduction
Nutrition is one of the oldest sciences known to humanity. Since ancient times, people have recognized the close relationship between food, health, and disease. Long before modern laboratories and scientific discoveries, early civilizations observed that certain foods could prevent illnesses, promote healing, and improve overall health. As a result, food was regarded not only as a source of nourishment but also as an important form of medicine.
The history of nutrition reflects the gradual transition from traditional beliefs and observations to evidence-based scientific discoveries. Over centuries, advances in medicine, chemistry, biology, and molecular science have transformed nutrition into a recognized scientific discipline and healthcare profession.
Nutrition in Ancient Civilizations
400 B.C.
Foods were often used as cosmetics or as medicines in the treatment of wounds. In some of the early Far-Eastern biblical writings, there were references to food and health. One story describes the treatment of eye disease, now known to be due to a vitamin A deficiency, by squeezing the juice of liver onto the eye — vitamin A is stored in large amounts in the liver.
Since earliest times, food has been considered in the light of both cause and cure of diseases, given its effect on health and productivity.
a) Ancient Civilizations
Ancient civilizations recognized that food was essential for life and health. Foods were commonly used as medicines, cosmetics, and remedies for treating wounds and illnesses. Although people did not understand nutrients scientifically, they observed that certain foods promoted healing and improved health.
b) Nutrition in the Bible
The Bible contains several references linking food with health and healing. For example, Daniel and his companions remained healthier after consuming vegetables and water instead of the king's rich food (Daniel 1:8–16).
c) Hippocrates (460–370 B.C.)
Hippocrates, the Father of Medicine, was among the first to emphasize the importance of diet in maintaining health and treating disease. He believed that diseases had natural causes and advocated the use of food as part of medical treatment. His famous statement, "Let food be thy medicine and medicine be thy food," remains a guiding principle in nutrition and dietetics.
Aristotle (384–322 B.C.)
Aristotle contributed to the understanding of biology and physiology. He recognized that food was necessary for growth, development, and maintenance of life. His studies helped advance knowledge about body functions and metabolism.
Era 1 — Chemical Analysis (1700–1900)
The Chemical Analysis Era marked the beginning of modern nutrition science. During this period, scientists studied the chemical composition of food and its relationship to health and disease.
- Dr. James Lind (1716–1794) — a British naval physician who conducted the first scientific experiment in nutrition. He observed that sailors on long voyages developed scurvy due to a lack of fresh fruits and vegetables; sailors given limes recovered from scurvy, demonstrating that citrus fruits could prevent the disease. This discovery later led to the identification of vitamin C as the nutrient responsible for preventing scurvy.
- Antoine Lavoisier (1743–1794) — known as the Father of Modern Nutrition and Chemistry, he demonstrated that the body produces energy by combining food with oxygen, explaining metabolism and laying the foundation for understanding energy production in the body.
- Justus von Liebig (1803–1873) — a German chemist who described the chemical composition of the major nutrients, showing that carbohydrates are composed of sugars, fats of fatty acids, and proteins of amino acids.
Era 2 — The Biological Era (1900–1955)
Characterized by the discovery of vitamins, minerals, and essential amino acids, and increased understanding of their roles in growth, development, metabolism, and the prevention of deficiency diseases. Scientists used laboratory animals and human studies to identify essential nutrients and establish recommended dietary requirements.
- Elmer Verner (E.V.) McCollum (1879–1967) — pioneered the use of laboratory rats in nutrition research at the University of Wisconsin. Discovered the first fat-soluble vitamin (Vitamin A) and later contributed to the discovery of Vitamin D, demonstrating the importance of vitamins in growth, vision, and bone health.
- Dr. Casimir Funk (1884–1967) — in 1912, coined the term "vitamins," proposing that certain substances in food were essential for life and could prevent deficiency diseases such as beriberi, scurvy, pellagra, and rickets.
- Sir Frederick Gowland Hopkins (1861–1947) — demonstrated that animals require small amounts of "accessory food factors" in food for normal growth and health, later identified as vitamins. Shared the 1929 Nobel Prize in Physiology or Medicine for this discovery.
- Christiaan Eijkman (1858–1930) — discovered that beriberi resulted from consuming polished rice lacking an essential nutrient, leading to the discovery of Vitamin B₁ (thiamine) and demonstrating that deficiency diseases could result from inadequate nutrient intake.
- Joseph Goldberger (1874–1929) — established that pellagra was caused by poor diet rather than infection, paving the way for the identification of niacin (Vitamin B₃) as the nutrient that prevents pellagra.
- Lucy Wills (1888–1964) — discovered the Wills Factor, later identified as folic acid (Vitamin B₉), after studying anaemia among pregnant women, greatly improving maternal nutrition.
- William C. Rose (1887–1985) — identified the essential amino acids, demonstrating that some amino acids cannot be synthesized by the body and must be obtained from the diet, establishing human amino acid requirements.
Era 3 — The Molecular and Cellular Era (1955–Present)
Represents the modern phase of nutrition science. Advances in molecular biology, biochemistry, genetics, and biotechnology have greatly improved understanding of how nutrients function at the cellular and molecular levels. Nutrients are now understood to not only provide energy and support growth but also regulate gene expression, metabolism, immune function, and numerous biochemical processes essential for maintaining health.
Greater understanding has also been gained of the roles of vitamins and minerals as components of enzymes, coenzymes, and hormones that regulate metabolic reactions. This era has seen the emergence of new fields such as nutrigenomics, nutrigenetics, and functional foods, which examine how nutrients interact with genes and how diet can be used to prevent and manage chronic diseases such as obesity, diabetes, cardiovascular disease, and cancer.
Nutrition and dietetics has evolved into a globally recognized healthcare profession, with registered nutritionists and dietitians now integral members of multidisciplinary healthcare teams alongside physicians, nurses, pharmacists, and other health professionals in health promotion, disease prevention, treatment, and rehabilitation.
History of Nutrition in Kenya
The development of nutrition in Kenya has evolved from traditional food systems and indigenous knowledge to a well-established profession supported by research, education, and government policies.
Early Development
Before the colonial period, Kenyan communities relied on indigenous knowledge to meet their nutritional needs. Diets consisted mainly of locally available foods such as cereals, legumes, vegetables, fruits, milk, meat, and fish. Communities practiced breastfeeding, complementary feeding, food preservation, and the use of medicinal plants to promote health and treat illnesses.
Colonial Period (1895–1963)
Nutrition services were introduced mainly through hospitals, mission health facilities, and agricultural extension programmes, focused on controlling nutritional deficiency diseases such as protein-energy malnutrition, vitamin A deficiency, iodine deficiency, and anaemia. Nutrition education was primarily targeted at mothers and children.
Post-Independence Development
After independence in 1963, the Government of Kenya recognized nutrition as an important component of public health. Nutrition programmes were expanded through the Ministry of Health to improve maternal and child nutrition, food security, and nutrition education. Universities and middle-level colleges also introduced training programmes in nutrition and dietetics.
Professionalization of Nutrition
A major milestone was the enactment of the Nutritionists and Dieticians Act, 2007 (Act No. 18 of 2007), which established the Kenya Nutritionists and Dieticians Institute (KNDI) — responsible for regulating the training, registration, licensing, and professional practice of nutritionists and dietitians in Kenya while ensuring adherence to professional standards and ethical conduct.
Nutrition in Kenya Today
Nutrition is recognized as a key pillar of healthcare and national development. Nutritionists and dietitians work in hospitals, community health programmes, schools, universities, research institutions, NGOs, food industries, and private practice — covering nutrition assessment, counselling, disease prevention, health promotion, food service management, research, and policy development.
The Government of Kenya continues to strengthen nutrition through initiatives such as the National Nutrition Action Plan (NNAP), food and nutrition policies, school feeding programmes, food fortification, micronutrient supplementation, and the promotion of optimal maternal, infant, and young child nutrition.
